Adult Case Manager – Jail Diversion – Helena
Part-Time Non-exempt Position – Helena
Duties: Carry a caseload as assigned by Supervisor in accordance with Detention Mental Health Standards. Assist clients to identify strengths, needs and goals in residential, health, vocational, educational, social, and economic domains. Meet assigned productivity standards regarding billable units. Coordinate necessary specialized services including psychiatric, psychological, medical, educational, and housing based on the needs and desires of the client. Involve family and significant others, with the appropriate releases, as needed in the assessment and planning process of service delivery. Assist in the development and implementation of written case management treatment plans to be signed by both treatment teams. Facilitate client access and utilization, education, and facilitation of advocacy groups in the community. Assist clients to acquire life skill education in a natural environment in order to maximize an independent lifestyle. Grant funded position until December 31, 2015. Position will be re-evaluated for further funding source. Bargaining unit position, must participate in the Union pension fund.
Qualifications: Minimum qualifications are a bachelor’s degree in a human services field is required and at least one year of full-time experience is preferred. Other education and experience may be considered. Must have a personal vehicle, current vehicle insurance, and valid driver’s license to transport clients. Travel is required.
